PHYSICIAN CREDENTIALING

Getting a job as a physician is more complex than getting most other types of jobs. There’s quite a lot of licensing, applications, and documentation needed to apply for a physician job and get started working.

APPLICATION SUBMISSION

-State Licensing
At least 4 – 6 months prior to start date
If applying for multiple states, use of FCVS is helpful.
-NPI number (when licensed). An NPI number is required for Medicare enrollment.
-Commercial Health Plans
At least 90 days prior to start date
-Medicare/Medicaid provider number
Enrollment at least 60 days prior to start date
For Medicaid, contact state

DOCUMENTATION

-Current CV Important: Gaps greater than 30 days must be explained.
-Copies of
All licenses (active and inactive)
Board certificates
Diplomas & training certificates
Malpractice insurance certificate(s)
-NPPES confirmation letter (if applicable)
-Complete list of addresses and dates for hospital affiliations and past employers
